1. To send your medical records from Ballard Natural Medicine to another clinic
   We process medical records requests via a questionnaire in the Charm patient portal. Please contact the Front Desk and we will send you the questionnaire to complete.

2. To have your medical records from another clinic sent to Ballard Natural Medicine
   Please let your provider know that they can fax any medical records you want sent to us to the fax number 206-432-9509. If you have your own copies of your medical records that you’d like to share with your doctor here, you can upload PDFs to your Charm patient portal documents section. Make sure to select the option to share with your doctor.

3. For previous patients of Virginia Morris at Rain City Integrative Med
   For patients seeking their medical records from their care with Dr. Morris at Rain City Integrative Med, please contact Morgan Records Management.
