Patient Portal
Current patients can log in to the Charm Patient Portal to access your Health Records.
The Charm EHR system includes a patient portal. A portal invitation is sent when new patient appointments are scheduled. We encourage all patients sign up for the portal as it will be used for completing questionnaires, billing and online payments, all online communication with your physicians and staff, and provide access to your medical records and treatments plans. The portal is HIPAA compliant and will be used for all written communication.
